Title: A review on cost analysis of patients undergoing haemodialysis and the need for mobile dialysis facilities to cut down burden of indirect cost

Abstract

The progression of chronic kidney disease (CKD) to End Stage Renal Disease (ESRD) in India is rapidly increasing due to lack of medical facilities, poor control of risk factors and delayed referral to nephrologists. Over 25million of the people suffering from ESRD, are not undergoing dialysis due to the factors such as lack of awareness of the disease, lack of treatment options and the affordability is hindered by low income, minor reimbursement for chronic illness.

While calculating the cost of haemodialysis in private hospitals, around Rs. 12000 per/head/month and 140000 per year. This seems to be expensive for the middle and lower middle class of patients. Many of the rural patients belongs to lower social economic status and opt government health care facility over private. Hence our study is aimed to analyse the healthcare cost of haemodialysis in government tertiary health care center with the objectives of analysing the direct and indirect cost of haemodialysis in tertiary health care center on patient perspective.

Data analyzed included and their demographic details, past and present medical history, cost per session of dialysis, Lab investigation, Money spent on travelling and the loss of days from work were collected. Out of 60 patients, 47 were male and 17 were female. 53 out of 60 were between age group 18-65. 38.33%, 23.33%, 21.66% and 8% were belong to lower middle, middle, lower and upper middle class respectively. 100% of the patients had completed and satisfied with the treatment. Average direct medical cost per head per dialysis was 481.5 INR and in direct medical cost were 557.33 INR.

With the above analysis, we would like to suggest that mobile dialysis centers would be beneficial in terms of cutting down the cost and this could also been tried to implement as a part of national health schemes.
