Title: Laboratory information systems for the internal quality control at cardiac surgery department

Abstract

Introduction: Since its foundation, cardiovascular surgery has been one of the most sophisticated parts of medicine. That is why everything that this specialty is associated with meets the highest requirements. Laboratory diagnostic plays an important role in the treatment and prevention of all known diseases. In recent years, Laboratory Information Systems (LISs) are increasingly being introduced into the routine practice of healthcare facilities in order to optimize it. 

Materials: In order to optimize the work at the National Medical Research enter of cardiovascular surgery, the laboratory information system. Innova system was introduced into routine practice in June 2017. 

Aim: The aim of our study was to analyze the effectiveness of laboratory diagnostic prescription in a cardiac surgery center through LIS introduction. Then control of feasibility of prescribing repeated laboratory tests was carried with the assessment of the economic component of the process. 

Methods: Descriptive parallel group study was conducted. Results are provided using descriptive statistics methods. 

Results: We have seen first result in decrease total number of laboratories prescribing in all studied tests groups and the cost reduction per patient. The total number of studies decreased by 20%, which is a good indicator, since when the value of the studies performed is close to 5000, this means 1000 omitted unnecessary laboratory tests and invasions. 

LIS introduction enabled to obtain a tool for quantitative and qualitative analysis of the feasibility of prescribing laboratory tests, as well as to conduct economic analysis of the results. The data obtained were further used in the development of laboratory profiles allowing to continue optimization of this area and implementation of lean technologies in a hospital.

Anastasia Shcherbak works as cardiac surgeon and deputy head of department of quality control of treatment at the National Medical Research Center of Cardiovascular Surgery in Moscow. Has been awarded with the third prize of the Cardiac Young Surgeon Session with the abstract “Midterm clinical outcome of right ventricular outflow tract reconstruction using the pulmonary homograft” during the 69th ESCVS Congress held on March 26-27, 2021.
